Ask the Experts
0:00:05.000,0:00:10.000
When should fertility treatment success rates influence which fertility treatment to use?
0:00:10.000,0:00:21.000
Dr. Simon Kipersztok, Shady Grove Fertility: Authorities have determined that a success rate of 5 percent or less signifies poor prognosis.
0:00:21.000,0:00:30.000
And a success rate of 1 percent or less is consistent with a futile attempt.
0:00:30.000,0:00:42.000
So, some practitioners have taken that recommendation by authorities to mean that if the probability of success with any given intervention
0:00:42.000,0:00:53.000
is less than 5 percent but higher than 1 percent, you may want to try it for one or two cycles, OK?
0:00:53.000,0:01:00.000
But the outlook is very poor in terms of the outcome that you are looking for.
0:01:00.000,0:01:11.000
In contrast, if, for example, there is a situation of a woman who is 42 years of age, with one blocked tube and abnormal sperm,
0:01:11.000,0:01:19.000
doing an IUI, in my opinion, under those circumstances, would yield a probability of conception that is 1 percent or less,
0:01:19.000,0:01:22.000
making it a futile attempt.
0:01:22.000,0:01:28.000
And I feel that it's important for patients to consider that information when making that decision,
0:01:28.000,0:01:38.000
because in this particular situation of the 42-year-old woman, when the probability is in the 1 percent range, time is lapsing,
0:01:38.000,0:01:44.000
and their ovarian reserve is getting poorer and poorer with time.
0:01:44.000,0:01:54.000
So, they are missing that window because of misinformation about the potential success rates of intrauterine inseminations.
