Can You Explain Pregnancy Rates with and Without Genetic Testing with PGS?

Dr. Bruce Albrecht, Albrecht Women’s Care, Denver IVF: If any doctor tells you it improves the pregnancy rate, the answer is absolutely it does not.
Because I want you to think about this. If a woman has four embryos that could be implanted, we have three possibilities of what we could do.
We could go ahead and transfer all four embryos and that might end up being exactly the right thing to do, because perhaps one of those will take.
On the other hand, perhaps all four would take. And if all four take we have really screwed up and so we don’t ever put in four.
But whatever numbers of embryos will take, if we put all four in that woman will have that number of babies.
Second possibility: Let’s go ahead and freeze all the embryos and transfer them one at a time over four different months. What would that accomplish? In the end, the same number of babies, because the embryos were either normal or they’re abnormal and so we may end up anywhere between zero and four babies. But it would be the same number of babies as if we put them all in at one time.
Third possibility: Let’s go ahead and do PGS. Let’s screen the embryos and discard the ones that are abnormal and only transfer the ones that are normal. How many babies do we end up with? Exactly the same number.
So, we’ve done nothing to improve the chances of having a baby. It does decrease the time to become pregnant, because in the scenario that I would do, I would transfer one embryo at a time. Freeze them all. Transfer one embryo at a time. And it might take four tries for a patient to achieve the same number of pregnancies that she could perhaps achieve in fewer tries if she did the PGS.
It will not be a hundred percent pregnancy rate and it will not be a zero percent miscarriage rate, but we can select embryos that are more likely to implant and less likely to miscarry.
